Out of frustration, this person interviewed elsewhere and landed a great offer. But hours later, they got the H-1B result where he got approved under the same employer they were ready to leave.

Now they’re stuck between two terrible options: stay with a miserable job just to keep the H1B, or leave for a better life and risk losing the visa next year.
The new company doesn’t allow deferrals so they need to take action immediately. Therefore, even premium processing might not help.
